Risdiplam is a small-molecule survival motor neuron 2 (SMN2) splicing modifier used for the treatment of spinal muscular atrophy (SMA) in pediatric and adult patients. SMA is a rare, inherited neuromuscular disorder characterized by progressive muscle weakness and atrophy resulting from the degeneration and loss of motor neurons in the spinal cord and brainstem. The disease is most commonly caused by mutations or deletions in the survival motor neuron 1 (SMN1) gene, which lead to insufficient production of functional survival motor neuron (SMN) protein. The severity of SMA varies considerably among patients and is influenced in part by the number of copies of the closely related SMN2 gene.

Risdiplam works by modifying the splicing of SMN2 precursor messenger RNA. Normally, the majority of SMN2 transcripts exclude exon 7, resulting in the production of an unstable and rapidly degraded SMN protein. Risdiplam promotes the inclusion of exon 7 in SMN2 mRNA transcripts, thereby increasing the production of full-length, functional SMN protein. Increased SMN protein concentrations help support the survival and function of motor neurons and can improve or preserve motor function in patients with SMA. Unlike gene replacement therapies or intrathecally administered antisense oligonucleotides, risdiplam is administered orally and is systemically distributed.

In August 2020, risdiplam was approved by the U.S. Food and Drug Administration (FDA) for the treatment of spinal muscular atrophy in patients two months of age and older. The approval was supported by clinical studies evaluating patients with infantile-onset and later-onset SMA, including the FIREFISH and SUNFISH clinical development programs. In patients with infantile-onset SMA, treatment resulted in the achievement of motor milestones that would rarely be expected during the natural course of untreated disease, while patients with later-onset SMA demonstrated improvements in measures of motor function compared with placebo. Risdiplam was the first orally administered drug approved by the FDA for the treatment of SMA.

In May 2022, the FDA expanded the indication for risdiplam to include infants younger than two months of age, resulting in its current indication for the treatment of SMA in pediatric and adult patients. It was initially available as a powder for oral solution, and a 5 mg tablet formulation was subsequently approved in the United States in February 2025. The tablet may be used in patients prescribed the 5 mg daily dose and can be swallowed whole or dispersed in water.
